Miami, Florida — Your First Visit at Palmetto Fertility Center

Thank you for your interest in our fertility program. Our Fertility Program is managed by Michael Graubert, MD, FACOG, and Rafael A. Cabrera, MD, FACOG, both Board-Certified Specialists in Infertility and Reproductive Endocrinology. We offer a warm, caring and supportive approach to fertility treatment that you may find different from other programs. All monitoring visits are done by our physicians which means you will have continual physician contact throughout your cycle.

Our office provides many fertility procedures including: IVF, inseminations, and reproductive surgery. We have excellent success rates with our couples. Our IVF success rates are among the best in Florida.

Insurance Coverage

Some insurance companies do NOT cover any fertility related physician visits. Other insurances cover fertility evaluations for the diagnosis of infertility, but NOT any treatments. Some rare insurances will cover all fertility visits and treatment including IVF! We will verify your insurance coverage on your first visit with us so you understand your benefit completely.

• Cigna HMO and PPO
• Humana HMO and PPO

If you do not have insurance, the cost of an initial consultation is $400, which includes a consult, an exam, and a pelvic ultrasound. Scheduling an Appointment

The Initial Consultation / Important Forms

Your initial consultation will last for approximately one hour. We welcome your partner at this visit as well.

Please complete the following forms and bring them to your first visit:

• Female Questionnaire
• Male Questionnaire
• Registration Form

Also, please complete a medical record release form and send it (fax is recommended) to your past physician(s) requesting your prior records. Your physician(s) will forward the records to Palmetto Fertility Center. Having all of this information available will greatly accelerate your infertility evaluation and treatment at our center.
