Thank you for your interest in the fertility program at Palmetto Fertility Center, with offices in Miami and Miramar, Florida. Our fertility program is managed by Michael Graubert, M.D., F.A.C.O.G., a board-certified specialist in Infertility and Reproductive Endocrinology. Palmetto Fertility Center offers a warm, caring, and supportive approach to fertility treatment that you may find different from other programs

Our office provides many fertility services, including IVF (in vitro fertilization), inseminations, and reproductive surgery. We have excellent success rates with our couples.

Insurance Coverage

We accept:

• All PPO insurances (we are out of network for Aetna PPO)
• Humana HMO and PPO
• Cigna HMO and PPO

• Neighborhood Health Plan
• Blue Cross/Blue Shield PPO
• Blue Options
• Health Options
• Vista Health Plan

Please be aware that many insurance plans do not cover significant portions of infertility services.

If you do not have insurance, the cost of an initial consultation is $500, which includes a consult, an exam, and a pelvic ultrasound.

Scheduling an Appointment

The Initial Consultation / Important Forms

Your initial consultation will last for approximately one hour. We welcome your partner at this visit as well.

Please complete the following forms and bring them to your first visit:

• Female Questionnaire
• Male Questionnaire
• Registration Form

Also, please complete a medical record release form and send it (fax is recommended) to your past physician(s) requesting your prior records. Your physician(s) will forward the records to Palmetto Fertility Center. Having all of this information available will greatly accelerate your infertility evaluation and treatment at our center.
