Palmetto Fertility Center IVF Advantage Program

JULY 1, 2009 TO DECEMBER 31, 2009

Despite the excellent pregnancy rates from IVF, some patients are concerned about the costs of having to repeat an IVF cycle if they do not conceive. We also understand that patients not eligible for our money back guarantee program and need a special program of their own. Therefore, we have begun a discount program that everyone will be eligible for!

Our IVF Advantage Program discounts each subsequent cycle of IVF you attempt if you do not conceive in the previous cycle. (This program is for patients using non-donor eggs). All patients who initiate their first IVF cycle between July 1 and December 1, 2009 will be eligible for this discount program.

Fees to Palmetto Fertility Center

IVF Global Fee, cycle #1..........................................................................................$10,000.00

1. Includes all ultrasounds, lab work during the cycle, office visits, egg retrieval, semen analysis, sperm preparation, fertilization of eggs, embryo preparation and embryo transfer.

2. Fee includes blastocyst culture and assisted hatching if applicable.

3. From July 1 to December 31, 2009, ICSI is now included.

4. Fee does NOT include PGD, embryo freezing, surgical sperm processing, infectious disease quarantine charge, anesthesia fee, or medications.

IVF Global Fee, cycle #2 if no conception............................................................$8,000.00

IVF Global Fee, cycle #3 if no conception............................................................$6,000.00

Additional Laboratory Fees per case

1. Intracytoplasmic sperm injection – ICSI (if needed)....................2,000.00 (now included)

2. Embryo Cryopreservation (includes 1 year of storage)...............1,000.00

3. Surgical sperm processing (testicular biopsy cases)....................500.00

4. Infectious disease quarantine charge (hepatitis positive).............500.00
