Palmetto Fertility Center IVF Advantage Program

MAY 1, 2010 TO DECEMBER 31, 2010

We understand that the high cost of IVF may stop you from repeating an IVF cycle, and prevent you from building your family. We also understand that patients not eligible for our money back guarantee program need a special program of their own. Therefore, we have begun a discount program that everyone will be eligible for! Our IVF Advantage Program discounts each subsequent cycle of IVF you attempt if you do not conceive in the previous cycle. (This program is for patients using non-donor eggs).

Fees to Palmetto Fertility Center

IVF Global Fee, cycle #1..........................................................................................$11,000.00

1. Includes all ultrasounds, lab work during the cycle, office visits, egg retrieval, semen analysis, sperm preparation, fertilization of eggs, embryo preparation and embryo transfer.

2. Fee includes blastocyst culture and assisted hatching if applicable.

3. Fee includes ICSI (intracytoplasmic sperm injection) if needed.

4. Fee now includes embryo freezing any remaining quality embryos after embryo transfer, including storage for 1 year. Only viable blastocysts are candidates for embryo freezing.

IVF Global Fee, cycle #2 if no conception............................................................$9,000.00

IVF Global Fee, cycle #3 if no conception............................................................$7,000.00

Our Global Fee applies to all patients paying out of pocket, with no insurance benefits for IVF. If you have IVF insurance coverage, you will be given our IVF insurance fee schedule to estimate your co-pays and deductibles

Note: Our Global Fee does NOT include special procedures that may be required in some cases, such as PGD, surgical sperm processing, annual frozen embryo or annual sperm storage fees or if hepatitis positive, a quarantine surcharge

1. Quarantine Fee (for infectious disease cases), per case....................500.00

2. Preimplanation Genetic Diagnosis – PGD per case, starting at...............4,200.00

3. Fee for ICSI of testicular sperm, per case....................500.00

4. Annual fee for embryo storage, after first storage year.............450.00
